Office Policies

 

First Visit

A $100 deposit is required to reserve a First Visit appointment with Dr John. This deposit counts toward first the full fee.

If a first visit is either missed, or cancelled, or rescheduled within a week of the scheduled appointment, this deposit is not transferable toward any other appointment.

Payment

Due at time of visit by credit card, debit card, check, or cash. Insurance billing forms are generated for patient use for insurance reimbursement.

Personal Injury Cases

Auto accident/personal injury cases are accepted, no lien.

Workers' Compensation Cases

Workers Compensation cases are accepted. Authorization is required prior to first visit.

X-ray and MRI

Not required for initial visit, and prescribed only if clinically necessary.

If you already have an X-ray and / or MRI, or written report of them, please bring to your first visit.

Cancellation or Re-scheduling an Appointment After First Visit

If an follow up appointment is cancelled or rescheduled, there may be a fee if the change occurs in the time frames below:

  • More than 24 hours notice: no charge
  • Less than 24 hours notice: $30
  • Less than 4 hours notice: $40
  • Missed appointment, no notice: $50
