Office Policies

First Visit

A $100 deposit is required to reserve a First Visit Appointment with Dr John. This deposit counts toward the first visit $225 fee. This can be accomplished with a phone call to our office, or by fulfilling a Stripe invoice that we can send.

If a first visit is missed or is cancelled within a week of the scheduled time, this deposit is not refundable or transferable. One exception would be if the appointment were changed to a day or time slot that is earlier/sooner than the original, if such a re-schedule is possible.

Remainder of first visit fee is due at time of visit by credit card, debit card, check, or cash. Insurance billing forms are generated for patient use for their insurance reimbursement.

Personal Injury Cases

Auto accident/personal injury cases are accepted as Dr John has much experience with med/legal requirements of personal injury. We do not, however, accept lawyer liens. We do provide all required documentation for one’s lawyer or insurance company.

Workers' Compensation Cases

Workers Compensation cases are accepted. Authorization is required prior to first visit.

X-ray and MRI

Not required for initial visit and prescribed only if clinically necessary.

If you already have an X-ray and / or MRI, and/or written report, please bring them to your first visit. The narrative report is most important.

Cancellation or Re-scheduling an Appointment

Below is a simplified summary of this document:

  • First visit appointment deposit: non-refundable / non-transferable if cancellation is made with less than one week to appt time.

  • Established patient appointment - cancel or re-schedule to a later appointment visit with less than 72 hours’ notice:

    • 45 min appt $120

    • 30 min appt $100

  • Established patient appointment - cancel or re-schedule on the same day of the appointment

    • 45 min appt $160

    • 30 min appt $120.

  • Changing appointment to an earlier time slot, if available, no charge.
